Terms of Service

Last updated: 18 June 2026 · Effective from: 18 June 2026

These Terms of Service ("Terms") are a legal agreement between you and Yourmedrecord Pte. Ltd. ("YMR", "we", "us", "our"), a company incorporated in Singapore (UEN [UEN]), registered office at [SINGAPORE REGISTERED ADDRESS]. They govern your access to and use of the YourMedRecord website, WhatsApp service, email service, web dashboard and related services (together, the "Service").

Please read these Terms carefully. By creating an account or using the Service, you agree to be bound by these Terms. If you do not agree, do not use the Service.

These Terms work together with our Privacy Policy, which explains how we handle your personal data, and with the consent you provide at onboarding. Where these Terms and the Privacy Policy address the same subject, the Privacy Policy governs how we handle your data.

YMR is a personal health record vault and conversational health-information assistant. It helps you store, organise, retrieve, summarise, explain and trend medical documents you choose to send it.

YMR is not a hospital, clinic, doctor, medical practitioner, emergency service, pharmacy, diagnostic laboratory, or licensed healthcare provider, and the Service is not a medical device. YMR does not provide medical diagnosis, treatment, prescriptions, emergency advice, or clinical decision-making.

Any summary, explanation, trend, extracted value, risk flag, interpretation, suggested question or specialty guidance the Service generates is provided for general information, record organisation and convenience only, and may be incomplete, inaccurate, mistranslated or outdated. Always consult a qualified healthcare professional before making any medical, treatment, medication, diagnostic or emergency decision. In an emergency, contact emergency services or attend the nearest hospital immediately.

Where an original medical report differs from any information the Service extracts, summarises, translates or explains, the original report prevails. You are responsible for verifying important information against your original records.

"Account" — the account you create to use the Service.

"Content" — the documents, reports, images, messages, files and other materials you submit to the Service, and the information extracted or generated from them.

"Registered Channel" — the mobile/WhatsApp number and email address registered to your Account.

"Subscription" — a paid or free plan tier you select.

"De-identified Data" — data processed so that it no longer identifies you, as described in the Privacy Policy.

You must be at least 18 years old and have the legal capacity to enter into these Terms. By using the Service you confirm that you meet these requirements.

To use the Service you must register and provide accurate, current and complete information, including a valid mobile/WhatsApp number and email address. We verify your contact details (for example, by OTP and email validation). You agree to keep your information up to date.

You are responsible for activity on your Account and for keeping your devices, WhatsApp account, email account and credentials secure. The Service processes Content only from your Registered Channel. Content sent from an unregistered number or email will not be added to your Account, and may instead receive a prompt to register. Notify us promptly if you believe your Account has been accessed without authorisation.

If you change your registered mobile number, your prior WhatsApp conversation history on the old number may be lost on WhatsApp, although your records remain available as described in the Privacy Policy and product documentation.

You may interact with the Service through WhatsApp, email and the web dashboard, as made available for your tier. You may send medical documents by forwarding or uploading them, or by photographing them. The Service confirms receipt and processes your Content using OCR and artificial intelligence to extract, classify, organise and explain it, and lets you retrieve and (through your own device) share your records.

The Service relies on third-party platforms — in particular WhatsApp (operated by Meta) — and on your network and device. Features, availability and behaviour may vary by channel, tier, region and over time.

6.1 Plans. The Service offers a free tier and paid tiers (currently Basic, Premium and Family). The features and limits of each tier — including document allowances, AI conversation/interaction limits, retention periods and conversation recall — are set out on our pricing page at XXXXXX which forms part of these Terms. We may vary tier features and limits; material changes are handled under Section 14.

6.2 Fees and currency. Fees for paid tiers are stated on the pricing page [in LKR / and other local currencies as applicable]. You authorise us and our payment providers to charge the applicable fees for your chosen tier.

6.3 Billing and renewal. Paid subscriptions are billed [monthly / annually] and renew automatically at the end of each billing period at the then-current price, unless you cancel before renewal.

6.4 Payment providers. Payments are processed by third-party providers (for example, [PayHere, LankaQR, Koko, Stripe, PayPal]). Your use of them is subject to their terms. We do not store full card details.

6.5 Cancellation. You may cancel at any time. Cancellation takes effect at the end of the current billing period; you retain access to your tier until then.

6.6 Refunds. Except where required by applicable law, fees are non-refundable, and we do not provide refunds or credits for partial periods or unused allowances.

6.7 Price changes. We may change fees. We will give you reasonable notice before a change affects you, and the new price applies from your next renewal. If you do not accept the change, you may cancel before it takes effect.

6.8 Taxes. Fees are exclusive of applicable taxes unless stated otherwise; you are responsible for any taxes that apply to your purchase.

The free tier is provided "as is" and may have reduced features, lower limits and shorter retention, as set out on the pricing page. We may modify, limit or discontinue the free tier at any time.

8.1 You own your Content. We are the keeper of your records, not the owner. As between you and us, you retain all rights in your Content.

8.2 The licence you give us. To operate the Service, you grant us a worldwide, non-exclusive, royalty-free licence to host, store, process, reproduce, analyse and display your Content, and to use OCR and artificial intelligence on it, solely to provide and improve the Service to you and to create and use De-identified Data, in each case as described in the Privacy Policy. This licence ends when your Content is deleted, except for De-identified Data and any copies we must retain by law, as described in the Privacy Policy.

8.3 Your responsibility for Content. You are responsible for the Content you submit and for ensuring it is lawful and that you are entitled to submit it. You confirm that information you provide is, to the best of your knowledge, accurate — the Service's outputs depend on the accuracy of what you provide.

8.4 Records relating to other people. If you submit or manage records relating to another person (for example, under the Family tier or as a caregiver), you confirm that you are authorised to do so and have obtained any consent required by law. You are responsible for who you give access to your Account or records; only authorise people you trust.

You agree not to:

  • use the Service for any unlawful, fraudulent or harmful purpose;
  • upload Content you are not authorised to upload, or that infringes any third party's rights;
  • upload malware, or attempt to gain unauthorised access to, disrupt, overload, reverse-engineer, scrape or circumvent any part of the Service;
  • misrepresent your identity or impersonate another person;
  • use the Service to provide medical advice to third parties or hold it out as a medical service;
  • resell, sublicense or commercially exploit the Service without our written consent; or
  • use the Service in a way that breaches the terms of WhatsApp, Meta or any other third-party platform.

We may investigate and take action — including suspending or terminating your Account — for suspected breaches.

The Service uses artificial intelligence, OCR and automated processing, provided in part through third-party providers acting as our processors, as described in the Privacy Policy. AI and OCR outputs may contain errors and are not a substitute for professional medical judgement (see Section 1). You acknowledge the limitations of automated processing and agree to verify important information against your original records and a qualified professional.

The Service operates through, links to, or integrates with third-party platforms and services (including WhatsApp/Meta, email providers, payment providers, and others). Your use of those services is governed by their own terms and privacy policies, and we are not responsible for third-party platforms or services we do not control, including their availability, security or data practices. Your communications with the Service may pass through systems we do not own or control.

Our collection, use and handling of your personal data is governed by our Privacy Policy and the consent you provide. By using the Service you confirm you have read the Privacy Policy. The Privacy Policy describes, among other things, how we use De-identified Data, our retention periods, and the process that applies if your Subscription lapses.

We aim to provide a reliable Service but do not guarantee that it will be uninterrupted, error-free, secure, or available at any particular time. The Service depends on third-party platforms (including WhatsApp) and your network and device, which we do not control. We may modify, suspend or discontinue all or part of the Service, perform maintenance, or impose or change limits, and we will give reasonable notice of material adverse changes where practicable. We may suspend your access where we reasonably believe it is necessary to protect the Service, other users, or to comply with law.

We may update these Terms to reflect changes to the Service, our operations, or the law. If a change is material, we will give you reasonable notice (for example, through the Service, WhatsApp, email or the website) before it takes effect. Your continued use of the Service after the updated Terms take effect means you accept them. If you do not accept a change, you may stop using the Service and cancel any Subscription.

Your records are retained for the period applicable to your tier, as described on the pricing page and in the Privacy Policy.

If your Subscription lapses or payment fails, a staged process applies: we notify you through your Registered Channel and the dashboard; if the Subscription is not reactivated within the stated period, access is suspended; and after a further defined period, your records are archived and then, following a final notice, deleted or irreversibly de-identified, as described in the Privacy Policy. We may retain certain data longer where required by law. It is your responsibility to keep your contact details current so you receive these notices, and to download any records you wish to keep.

By you: you may stop using the Service and close your Account at any time, subject to Section 6.5 (cancellation) and Section 15 (retention and deletion).

By us: we may suspend or terminate your Account, with notice where practicable, if you materially breach these Terms, use the Service unlawfully, or where required by law. We may also terminate on reasonable notice if we discontinue the Service.

On termination, your right to use the Service ends. Sections that by their nature should survive (including Sections 1, 8.2–8.4, 17–20, 22 and 23) survive termination.

To the maximum extent permitted by law, the Service is provided "as is" and "as available", and we disclaim all warranties not expressly stated in these Terms, including implied warranties of merchantability, fitness for a particular purpose, accuracy, and non-infringement. We do not warrant that the Service will meet your requirements, be uninterrupted or error-free, or that AI/OCR outputs will be accurate or complete. The Service is not a substitute for professional medical care.

To the maximum extent permitted by law:

  • We are not liable for any indirect, incidental, special, consequential or punitive damages, or for any loss of profits, data, goodwill or anticipated savings, arising out of or relating to the Service or these Terms.
  • We are not liable for any medical decision, action or outcome based on information from the Service, or for the acts, errors or content of third parties (including healthcare providers, WhatsApp/Meta, and payment providers).
  • Our total aggregate liability arising out of or relating to the Service or these Terms is limited to the greater of the amount you paid us for the Service in the [12] months before the event giving rise to the claim.

Nothing in these Terms excludes or limits liability that cannot be excluded or limited under applicable law (for example, liability for fraud, or for death or personal injury caused by negligence where such exclusion is not permitted).

You agree to indemnify and hold us harmless from claims, losses and expenses (including reasonable legal fees) arising from your breach of these Terms, your misuse of the Service, your Content, or your submission of records relating to another person without proper authority.

These Terms are governed by the laws of Singapore. Any dispute arising out of or in connection with these Terms or the Service is subject to [the exclusive jurisdiction of the courts of Singapore / arbitration in Singapore administered by the [SIAC] under its rules]. We encourage you to contact us first so we can try to resolve the matter directly.

Entire agreement: these Terms, the pricing page, the Privacy Policy and your consent form the entire agreement between you and us regarding the Service.

Severability: if any provision is found unenforceable, the rest remains in effect.

No waiver: our failure to enforce a provision is not a waiver.

Assignment: you may not assign these Terms; we may assign them in connection with a merger, acquisition or sale of assets, subject to the Privacy Policy.

Force majeure: we are not liable for failure or delay caused by events beyond our reasonable control.

Notices: we may give notice through the Service, WhatsApp, email or the website. Notices to us should go to [contact].

Language: if these Terms are translated, the [English] version prevails in case of conflict.
